A continuing care retirement community with all levels of care.  From free-standing independent living villas to full healthcare, we can meet your needs! Heritage Pointe of Fort Wayne is a continuing care living center that welcomed its first residents in January 2015. Nearly 90,000 square feet, the living center includes all levels of care - independent and assisted living apartments, healthcare, rehabilitation, and memory care.

The Villas at Heritage Lakes is a 65-and-over free-standing, independent-living villa community located right on our campus. Six floor plans, two and three bedroom models, two baths, two car garages. Our club house with fitness room, kitchen/coffee bar and gathering room is completely accessible at all times by our residents. Four lakes and nearly a mile of paved walkways add to the beauty of the neighborhood.

We are the third location of Heritage Pointe Communities, and proudly affiliated with Heritage Pointe of Warren, IN and Heritage Pointe of Huntington. All Heritage Pointe Communities locations are owned and operated by the United Methodist Memorial Home (UMMH).

Due to the recent influx of flu cases in the Allen County Area, the Health Commissioner has recommended that all extended health care facilities limit visitors. We are recommending and strongly encouraging that only 1-2 healthy adults visit during this time.
