Which adverse pregnancy outcomes increase with maternal age? A recent Danish national study published in Obstetrics and Gynaecology March 2018 has highlighted the increased risks associated with maternal age. It is well known that there is increased risk for foetal chromosomal abnormalities in older mothers, however the risks for other adverse outcomes during pregnancy are less well-defined. The investigators in this study collected data on over 350,000 pregnant women in Denmark from 2008 through 2014. The overall risk for adverse outcomes (chromosomal abnormalities, congenital malformations, stillbirth, miscarriage, preterm birth) was 5% (mothers age range-20 to 34 years), 7% (mothers age range-35 to 39 years) and 11% (mothers age range over 40 years). The additional risk amongst women in the over 40 years age bracket was mainly driven by increased probability of chromosomal abnormalities (7 times greater) and miscarriage (4 times greater) than those women aged 20 - 34. When the data was analysed adjusting for BMI, smoking, numbers of babies before and IVF there was no big difference. The encouraging thing from this particular study is that asides from chromosome abnormalities the risk of adverse pregnancy outcomes is relatively low even for women over 40 years of age. Although this is good news for women who might be considering postponing pregnancy, it still highlights the risks of miscarriage and chromosomal issues that are more likely in this older age group.
