Acute Low Back Pain – Physiotherapy Benefits and Flare-Ups’ Risk Factors Though `staying as active as possible` is one of the most common advice that recent practice guidelines offer to people suffering from low back pain, patients can have different beliefs about activities that may or may not flare-up their symptoms. (1) In 2018 (2) a study followed-up patients with acute back pain and found different activities and factors interfering on patient’s symptoms: • Prolonged sitting (over 6 hours) was the only activity significantly associated with flare-ups of symptoms. Contrary to common patient’s beliefs, lifting, repeated lifting, prolonged standing, running/jogging and non-contact sports were not associated with increased risks of worsening the symptoms; • Stress and depression were correlated with augmented risks of flare-ups; • People undergoing physiotherapy treatments had reduced chances of flare-ups. According to a systematic review of 2019 (3), the early intervention of a physiotherapist in patients with acute low back pain may reduce the costs of treatment, the subsequent health services utilization, the use of opioids and spinal injections and the number of surgical procedures, improving health care efficiency. Migraine? Physiotherapy and Acupuncture are Effective Options! Acupuncture has shown again its efficacy on the prevention of migraine, on a systematic review published in 2019, assessing almost 5.000 patients’ treatment results. (1) Patients treated with acupuncture had similar to superior effects on preventing migraine than patients receiving the standard prophylactic medication, with fewer side effects and with a reduced treatment drop out. Physiotherapy techniques (e.g. like muscle treatments, joint mobilization/manipulation, amongst others) (2) and aerobic exercises (3) have shown positive results modulating migraines as well. Neck and Back Pain: Direct Access to your Physiotherapist Can Reduce Treatment’s Costs Neck and back pain are frequent conditions that may affect people’s lives significantly – they are amongst the top 4 conditions leading to disability, not only in Australia but all over the world.1 A recent research (February 2018) published in the USA showed that patients who directly accessed Physiotherapy services had the same benefits than the ones that went first to a GP and after got a referral, but with a significant reduction on treatment costs.2 These results can be related to the quicker initiation of physiotherapy and the common adoption of active treatment strategies – frequently advised by physiotherapists, as well as with a reduction on requests for imaging and advanced treatments, often unnecessary.2,3 Physiotherapists are well resourced primary care providers, and in musculoskeletal settings have been held responsible for the initial assessment and referral to the most effective treatment in partnership with other healthcare providers, presenting high-efficiency and safety on their assessments and treatments proposed.4-6 The latest Clinical Practice Guidelines for Back7 and Neck8 Pain promote patient’s management by physiotherapists and their broader spectrum of assessment and treatments, like manual therapy and spinal manipulation, acupuncture and dry needling, exercises and patient education/reassurance, amongst others.
