Cardinal Woods Skilled Nursing & Rehab is a Legends Health Care Facility in Madison, OH Cardinal Woods is a 120 bed facility that is in a beautiful wooded location in Madison, OH. We accept Medicare, Medicaid, and most Insurance. We are a member of the Ohio Health Care Association. Cardinal Woods offers private and semi-private rooms with cable TV, in-room phones, and internet access.
MISSION STATEMENT
While residing at our facilities, you will be cared for by a team of highly qualified physicians, nurses, therapist, and state tested nursing assistants. Your stay will be enhanced with services from our registered dietician, licensed social worker, respiratory therapist, and dedicated activity professionals. Our dedicated staff is totally committed to helping each individual achieve their highest functional ability.

Please join us Saturday, September 9th for our craft and wellness fair. Cholesterol and other health screenings will be provided by Lake Health. We will have live music with Fred Barringer from 12-2, Doterra, 31, Mary Kay, Saffire Rose, LuLuRoe, Scentsy and a group of local Crafters.
Cinco De Mayo is almost here and what a beautiful way to celebrate it !
