Description
The Four Counties Family Health Team provides primary health care services in a team approach which brings together your family physician with other health care providers in order to provide to you the best possible care. The focus is on keeping you and your family healthy, not just treating you when you are sick.
Family Health Teams focus on chronic disease management, disease prevention and health promotion, and work with other health care organizations, such as public health units and Community Care Access Centres.
