We are open 24/7, with nurses covering the daytime hours on a variety of shifts, sharing some weekend work would be expected. We are seeking Team players with integrity and fantastic professional work ethics. Do you enjoy staying skilled across all aspects of nursing? If so read on- Our successful candidates will be seeing a variety of cases including acute presentations, QAS drop-offs, immunisations, pilot and dive medicals, chronic disease management involving CDM item numbers, and much more. This role involves relationship building and rapport with patients being paramount. Preparation of healthcare plans alongside the GP, creating thorough notes, be administratively accurate. Strong autonomy for the right person. Do you enjoy Chronic Disease Management? We are seeking a Practice Nurse who is skilled in Chronic Disease Management, conducting Health Assessments, GP Management Plans, Team Care Arrangements, and Mental Health Plans, creating great patient outcomes by working alongside the GP and connecting the patient to valuable community allied health support.
