It's about time we put into context how much 1ml is. Pretty much a few raindrops. A teaspoon you stir your tea or coffee with is 5ml. So is a kids Calpol medicine syringe or spoon(5ml). So chica, you're likely going to want another sitting for your lips alone after the first millilitre, never mind the rest of your face. Be realistic. Some of the best work we've done, the best before and afters in our gallery, those you scream raptures and platitudes about - are from at least 4 ml of dermal filler, well placed in smart and safe areas, by a medical expert. We can't magically create perfect results with a few raindrops, and anyone that says they can is lying. It's. Not. Possible. Trust what your medical experts tell you. If you want results, you may well need a few sittings, which will of course boost the longevity of the fillers themselves (as robust clinical studies have repeatedly shown). This photo from today in Dublin, follows one of Dr Dallas' safety tests during a liquid rhinoplasty, called "aspiration" . What is aspiration and what does it mean? This is only relevant for dermal fillers rather than Botox and refers to fixing our needle in the position we are about to inject, pulling back on the plunger of the syringe and ensuring no blood has pulled back into the syringe. If there is blood, this would imply that we might be in a blood vessel and hence not safe to inject. This is exactly what happened here, and is a red flag it's not safe to proceed and to remove the needle, place elsewhere and repeat, until safe to continue. What would happen, if Dr Dallas hadn't aspirated, and gone ahead and injected? Quite possibly, the subsequent injection would have blocked the artery and caused significant problems. Intra-arterial injections can block the flow of blood to the tissue, potentially leading to skin necrosis, ulceration, loss of function and scarring. Embolisation of filler can even enter the central retinal artery and cause blindness. Anything we can do to minimise this risk could be argued worthwhile. Furthermore, just because we get a negative aspiration test, doesn't mean we don't proceed with caution. A false sense of security and gung-ho approach can be even more dangerous. We also use cannulas where we can, and we check capillary reflow and aspirate constantly, in order to make sure we are safe. We also know exactly how to spot an issue, and would be able to dissolve our fillers instantly to avoid problems. Even the merest sniff of an issue, and we would instantly reverse. It's vital, and possibly why we haven't had a single vascular occlusion or compromise for the nose, in the history of the clinic. Nothing is without risk, but risk levels can be very different in the hands of varying injectors and the safety techniques they employ.

When Botox and Aliaxin filler are used for maximum effect. Sometimes naturally plumping out the lips with a little filler isn't enough to make the mouth as perfect as it could be. A gummy smile can make a patient feel self conscious (although we do like the little nuances that make you, you! It's all down to individual choice however). In a significantly large proportion of patients, it's the top lip that is "a problem". It either entirely disappears when they smile, it curls under when the patient makes certain expression, it's too thin in proportion with the lower lip, or the lateral volume (volume at the sides of the lip, is significantly lacking). A lot of lip issues are to do with hypo or hyper toned muscles, and not just a tissue deficit. This means that filler alone won't solve the problem. This patient in NYC had extremely strong levator or lifting muscles either side of her nose - which means a large gum show and that the lip is pulled extremely taut upon smiling. In these cases, Botox is administered to make the muscles "sleepy" enough, to be the same strength as is normal. This means that the lip isn't stretched quite so tight upon smiling or laughing, and filler is the icing on the cake - adding volume and shape. These were the eventual results of a correction after the first injector had injected above the lips (not from our clinic!).
