Pay to Win Raffle Promo 2017 Raffle Draw Mechanics: 1. This Raffle Promo is open to all DCSI Subscribers who pay their Bills on time without history of Disconnection-Pass due for the past 6-months. (1 raffle stub = 1 subscriber). 2. All raffle stubs should be legibly filled out with complete name, address, contact number/s and signature by the subscriber itself. (Your DCSI Account Information). 3. Raffle drop box is located at the DCS Office. 4. After validation, the Finance Dept. Representative will insert the filled-out stub in the secured and sealed drop box in the presence of the subscriber (payer). 5. This Promo runs from Nov. 4 to Dec. 15, 2017. 6. Cut-off of joining is until 12:00 pm, Dec. 15, 2017. 7. Disqualification: Must not be an employee of DCSI, originally us or any of our related partners for this raffle draw, our relatives up to second degree of consanguinity or affinity and other party who directly involved in organizing or conducting this draw. 8. Raffle draw will be held on December 15, 2017, 1:00 pm at the Dasca Cable Services, Inc. Office address at DCS Bldg., D. Mangubat St. Zone II, City of Dasmariñas, Cavite at the presence of DTI Representative. 9. Prizes are not convertible to cash. 10. A subscriber can only win once. 11. Winners will be notified by phone/email and a letter; list of winners will be posted in the DCSI website and Community Channel. 12. All winners will be given sixty (60) days after the draw to claim their prize. After the 60-day period, unclaimed prizes will be forfeited in favor of DCSI Fund upon approval of DTI. 13. Winners must bring two (2) valid forms of identification such as driver’s license/SSS card/ Passport and the winning raffle stub (the other copy) in good condition upon claiming of their prize; in case of loss, winner must present the official receipt of payment. 14.
