The HSE have sent out a letter to all patients that still have a medical card or doctor visit card in Dr Hewetson's name asking them to fill in the Change of Doctor form. If you receive one of these letters you simply need to fill in the form and choose Dr Joanne Daly, then bring the form in to be signed here and we will look after the rest. You will be issued a new card in the name of Dr Daly and can continue to see any of the doctors working in the practice, including Dr Hewetson.
