PHN delivers efficient and cost effective Primary Health Care, with better integration across secondary care and other care transitions and interfaces. On 1 July 2015, the Australian Government established 31 Primary Health Networks to replace the existing Medicare Local system with the objectives of:
• increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
• improving coordination of care to ensure patients receive the right care in the right place at the right time.
The Darling Downs and West Moreton PHN is a joint venture between the Darling Downs Hospital and Health Service and GP Connections and is overseen by a Board of Directors and operated as an independent non-government private entity.
DDWMPHN works with GPs, allied and mental health professionals, practice nurses, the Hospital and Health Services and our key partners to develop and deliver a seamless healthcare system that fulfils the primary healthcare needs of our community.
The Darling Downs and West Moreton regions have a combined population of over 530,000 people with differing needs across the communities. From urban to rural communities, individuals and families, diverse cultural groups and Aboriginal and Torres Strait Islander people's, the Darling Downs and West Moreton PHN, knows that a 'one size fits all' approach to primary health care simply does not work. Through genuine and active engagement, education and training opportunities and partnerships, we will continue to work in the Darling Downs and West Moreton areas to ensure accessible and appropriate primary health care

A new service is now available to assist people managing diabetes after hours in our region! “Diabetes doesn’t keep 9 – 5 hours. It is a 24/7 condition” – Lynne McCleary, Credentialled Diabetes Educator, Manager AH Diabetes. Darling Downs & West Moreton PHN, in conjunction with AH Diabetes has established an After Hours service offering support and education to people managing diabetes, running from February to 30 June 2017. The service is available free of charge to people within the DDWMPHN region in After Hours periods only – from 6pm to 9pm on select days. An appointment based clinic will operate in Toowoomba on Monday and a drop-in clinic on Tuesday nights and an appointment-based rural tele-health line will run on Wednesday and Thursday nights. Darling Downs Hospital and Health Service Cultural Audit 2016 - Community Survey

The Darling Downs Hospital and Health Service (DDHHS) Cultural Audit Tool has been developed to evaluate the Cultural Capability in DDHHS. We ask both staff and community questions relating to the following: - Valuing Culture - Leadership and Accountability - Building cultural capability to improve economic participation - Aboriginal and Torres Strait Islander engagement and stronger partnerships - Culturally responsive systems and services These areas are aligned to the DDHHS Cultural Capability Plan that is put in place each year. DDHHS want to know how both the staff and community view the Cultural Capability of our services and which areas need improvement. Each DDHHS facility is given a target of audits to be completed based on staff number and community numbers. This audit tool allows Aboriginal and Torres Strait Islander community members an opportunity to have a say in the Cultural Capability of DDHHS and how our services are delivered in their local area. Recommendations from the Cultural Audit Report are then included in the DDHHS Cultural Capability Plan as actions and performance measures for the following year. - THE GUNDY MEDICAL MUSTER 2016 - Friday 9 September - Sunday 11 September REGISTER ONLINE: https://www.ivvy.com/event/GUND16/ MEDICAL EDUCATION WITH A FOCUS ON LIFESTYLE: The Medical Muster has been developed by local clinicians, and primarily aims to provide education and networking for rural health professionals. This weekend is fast becoming one of the highlights of Goondiwindi’s social calendar. The muster will be held alongside many of Goondiwindi’s premier events such as Rotary Gourmet in Gundy, Goondiwindi Aero Clubs Fly-in breakfast, Junior Golf Classic, Goondiwindi Cup Race Day, and the Goondiwindi Gun Club 2-day clay shoot. The full program provides the opportunity for partners and children to attend ‘Goondiwindi Cotton Tours’, ‘Gourmet in Gundy’, ‘the Beer and BS tour' at the Victoria Hotel, PCYC Goondiwindi activities, and Cheryl Moggs’ new Bush Tucker Tour just to name a few. The family program hopes to showcase what the country area has to offer and encourage medical professionals to consider the bush as a great career option. The program is coordinated by a committee of local Goondiwindi health professionals with support from Health Workforce Queensland (HWQ) and Darling Downs and West Moreton Primary Health Network (DDWM PHN).
