Eastern Melbourne PHN
Description
Eastern Melbourne Primary Health Network (PHN) works to improve the primary health care system for 1.5 million people in Melbourne's east and north-east. Eastern Melbourne Primary Health Network is one of 31 PHNs established by the Federal Government to facilitate improvement and integration initiatives within the primary health care system.
We cover a population of around 1.5 million people and an area of nearly 4,000 square kilometres across Melbourne’s eastern and north eastern suburbs and adjoining towns.
